1. Nature of Services I acknowledge that the services provided by FerFit Dietetics and Nutrition are nutritional and dietetic in nature. I understand that [Name of Practitioner] is an Accredited Practising Dietitian (APD) and that the advice provided is based on current evidence-based practice and the information I provide regarding my health and medical history.
2. Client Responsibility & Disclosure I agree to provide accurate and complete information regarding my medical history, allergies, medications, and physical condition. I acknowledge that failure to disclose such information may result in adverse outcomes and that I am responsible for updating my dietitian on any changes to my health status.
3. Assumption of Risk I understand that any significant change to diet or nutrition carries inherent risks (e.g., digestive changes, interactions with medications). I voluntarily choose to participate in the recommended program and assume responsibility for the risks associated with these lifestyle changes.
4. Medical Disclaimer & Coordination of Care I acknowledge that:
Dietetic services do not replace the advice of my Primary Care Physician (GP).
I am encouraged to consult my GP before starting any new supplementation or radical dietary shifts, especially if I have a pre-existing medical condition.
The dietitian may, with my consent, communicate with my other healthcare providers to ensure safety.
5. Limitation of Liability (Australian Consumer Law) To the extent permitted by the Competition and Consumer Act 2010 (Cth) and the Civil Liability Act of my state/territory:
I agree that the liability of FerFit Dietetics and Nutrition for any breach of a consumer guarantee is limited to the re-supply of the services or the payment of the cost of having the services supplied again.
This release does not exclude, restrict, or modify any non-excludable statutory rights or remedies available to me under the Australian Consumer Law.
6. Privacy & Confidentiality I consent to the collection and use of my personal and health information in accordance with the Privacy Act 1988 (Cth) and the FerFit Privacy Policy.
